Peer Support Specialist Certification: successful completion of the certification process within one year of employment Valid California Driver's License How you will make a difference: Provides services to clients within the community and/or at assigned sites, providing identified practices into services. Provides one-on-one support, mentoring, and role-modeling to clients and/or family members. Helps clients navigate peer support, wellness services and navigation supports within the behavioral health system of care, other health systems, and community supports. Links clients to various resources, including housing, food, education/vocation, recreation, spiritual, mentors, mental and physical health, and social, as deemed appropriate in partnership with the team. Collaborates with Treatment Team members on the development of plans of care and safety plans Provides direct services, including skill development, to individuals and peers. Assists clients with communicating with other providers and agencies. Advocates on behalf of clients and promotes self-advocacy. Provides and documents services as determined necessary by the team and within the Peer scope of practice. Documents services related to specific domains from the client’s perspective.
