New Horizons is seeking a caring and compassionate Peer Specialist who is eager to learn how they might provide support to clients who need assistance as they learn and grow. As a Peer Specialist your responsibilities will include: Provides peer support services either in office or in community for an assigned group of clients monitoring the activities of Care Coordination for high utilizing clients of New Horizons services Assists the Care Coordination Team with activities for assigned clients Prepares written documentation for each service conducted Provides transportation to consumers as needed Requirements New Horizons will request the following qualifications: Certified as a Peer Specialist or in process of Certification preferred Valid and current Florida Driver's License required with no more than 6 points in the past 6 years Must pass Level 2 Background through DCF and AHCA Clearinghouse.
