Overview To provide education to patients regarding medications, diet and self management of diabetes and associated conditions, such as comorbidities. Responsibilities - Establish and maintain positive relationships with patients, visitors, and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of BronxCare Health System and its employee. - Contributes to and participates in the Performance/Quality Improvement activities of the assigned department. Contribution and participation includes data collection, analysis, implementation of and compliance with risk management and claims activities, support of and participation in Continuous Quality Improvement (CQI) teams, consistent adherence to the specific rules and regulations of the BronxCare Health System (a) Safety and Security Policies, (b) Risk Management: Incident and Occurrence Reporting, (c) Infection Control Policies and Procedures and (d) Patient and Customer Service. - Assists in the planning and implementation of education projects targeting diabetes and other chronic diseases. - Conducts clinical responsibilities consistent with all clinical practice and job specific policies and procedures. - Assist director of ambulatory care in planning and coordinating patient education materials and events. - Maintain all necessary credentials to maintain certifications. - Assist director of ambulatory care in education and other elements of management of clinical aspects of care. - Directly educate patients about specific medical conditions (including diabetes, hypertension and others), including medication management, diet, prevention and other associated conditions and factors. Qualifications - At least 1 year of experience with hands on behavioral modification, medication management, and dietary management. - Certified Diabetes Care Education Specialist and Registered Dietitian Nutritionist - Bachelor’s in Nutrition - Master’s - Basic computer skills including the use of Microsoft word
