Role Snapshot Transform bedside care with AI. As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments. This is a per-diem, in-person position at skilled nursing facilities with flexible hours, requiring weekday availability only. \n What You'll Do Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers. High-volume virtual visits: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts. Quality & product feedback: Surface insights to improve workflows, summaries, and flagging logic; participate in root-cause reviews. Regional flexibility: Cover nearby sites (30–40-mile radius) as needed; mileage and lodging covered per policy. How Success is Measured (KPIs) Complete a minimum of one visit per hour worked to ensure compliance with regulatory standards. Maintain accurate and up-to-date documentation for each patient visit. Quaifications Certified and licensed Nurse Practitioner in TN; ANCC (or equivalent) in Adult/Family/Geriatric/Acute. Eligible for Medicare participation; company bills on your behalf. 2+ years NP experience with complex adult populations; able to practice independently. Valid TN driver’s license; comfortable traveling locally. Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools. Thrives in a dynamic, evolving startup environment. English required; Spanish preferred. \n Compensation This is a pay-per-visit role. Compensation will be based on each completed and signed clinical note in our system. Most practitioners are able to complete multiple visits per hour, allowing for competitive earnings based on efficiency and availability. Rate: $25/completed visit Onboarding/Meetings: Time spent on onboarding or attending meetings that are pre-approved by your manager will be compensated at a fixed rate of $64/hr. Schedule: Roughly 10 days per month, weekdays only.
