Description
Reviews and interprets medication orders, evaluates the patient’s drug regimen relative to the disease state, allergies, drug and food interactions and pertinent laboratory data prior to preparation and dispensing.  Interacts with prescribers to ensure accuracy, completeness and appropriateness of prescribing or ordering across all age groups represented by the population served.  Monitors drug therapy in conjunction with prescribers, nurses and other healthcare professionals to promote safe, effective therapeutic regimens.  Supervises activities of Pharmacy Interns, technicians and students. Qualifications * Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited Bachelors (BS) or Doctor of Pharmacy (Pharm.D.) Degree Program. * Completion of an American Society of Health-System Pharmacists (ASHP) accredited Pharmacy Residency Program – Preferred * Minimum hospital experience of one (1) year is preferred, with some experience in unit dose distribution, IV admixtures, clinical assessment and patient education. * New York State Licensed Registered Pharmacist * Knowledge of appropriate aseptic technique. * Knowledge of population specific medication dosing. * Knowledge of pharmaceutical calculations specific to drug dosing and compounding. * Proficiency in Microsoft office computer applications. * Proper written and oral communication skills.
