Lastly, we commit to increasing access to culturally responsive care beyond our walls through provider training and education, and policy advocacy.    Job Summary The Per Diem Registered Nurse (RN) helps fill-in when there are shortages on the nursing team and as such is responsible for providing clinically appropriate nursing care to Lyon-Martin clients within the scope of their license and credentials through clinic and telehealth visits, and phone communication. We expect the RN to act as a clinical leader, promoting holistic care, helping to grow the overall practice, and collaborating to optimize therapeutic outcomes. They are a critical member of the medical team, providing complex care management for patients with multiple chronic medical illnesses and behavioral health concerns and promoting team functioning and efficiency.  The RN, along with all staff at Lyon-Martin, are expected to uphold and imbue in others a commitment to addressing racism, ableism, fatphobia, gender and sexual discrimination within the organization and our clinical practice.    Key Responsibilities   Clinical Care Responsibilities  * Possesses strong clinical knowledge of and commitment to healthcare for trans, non-binary gender non- conforming, and intersex people, LGBQ communities, and cis-gender women * Provides patient care in accordance with clinical protocols and standing orders, guided by trauma-informed and harm reductionist principles * Applies motivational interviewing techniques to engage escalated patients and facilitate their care * Provides triage and nursing guidance to patients presenting in person or by phone with medical concerns * Facilitates clinic functioning and efficiency by taking responsibility for additional patient assessments or medical assistant functions (e.g., phlebotomy, rooming, vitals, etc.)  * Provides timely care coordination and management for medically complex patients (e.g., hospital transfer care coordination, procedure coordination)  * Assists with prescription issues, including submitting and following up on prior authorizations and helping trouble shoot pharmacy issues * Complies with all aspects of Lyon-Martin Policies and Procedures applicable to Staff members as well as all agreements with insurers and other entities that contain provisions applicable to or affecting the duties and responsibilities of Medical Staff; and all applicable statutes, regulations, governmental agency guidelines, and non-governmental oversight body requirements that place requirements on Lyon-Martin   Leadership Responsibilities:  * Provides clinical leadership to support staff, including medical assistants, patient care coordinators, front office staff, and receptionists * Incorporates constructive feedback into behavior change; participates as an active member of the care team through case-conferencing and other collaborative work * Demonstrates comfort in changing systems, champions change, and leads change processes   Performance and Quality Improvement Responsibilities:  * Completes mandatory trainings within deadlines * Helps support the quality assurance of medication supply, vaccine management, dispensary, point of care testing, laboratory, infection control    Organizational Responsibilities:  * Acts as a liaison between clinical and non-clinical staff * Keeps up to date with operational and procedural requirements * Supports the interpretation of standards and processes to meet best practice standards established in the health services program   And other duties as assigned    Competencies: Cultural Humility: Ability to work with a diverse population while withholding judgment. Takes a thoughtful approach when considering options; may seek supervision and/or input from others  Attention to Detail: Strives to eliminate errors; makes accurate work a priority; seeks opportunities to improve performance  Time Management: Organizes and establishes priorities; gets the job done in a timely manner  Customer Service: Persists in efforts to solve issues even when faced with internal barriers; takes personal responsibility for customer service outcomes; responds quickly and effectively to requests for assistance and support whether internal or external  Computer Literacy: Skilled computer-based work tasks; uses technology to enhance job performance.     Education, Knowledge, Skills: * Graduation from an accredited school of nursing * Possession of a valid California license as a registered nurse * Two years of direct patient care experience; one year in ambulatory care and/or a community health center preferred * Experience with electronic medical records * Comfortable working with, and committed to serving, safety net populations * Experience with and commitment to harm reduction and trauma informed care * Experience working with lesbian, bisexual, transgender, non-binary, gender non-conforming, and intersex clients * Commitment to working in an environment offering abortion services and the full scope of pregnancy options counseling * Experience working with populations with varying disabilities, racial, ethnic and cultural backgrounds and economic statuses * Experience working with clients who do sex work, those experiencing homelessness, people who have challenges with mental illness and/or are using substances, those with a history of incarceration, people of varying ages    Desired:  * A minimum of two years of nursing experience since graduation (excluding experience obtained during nursing school), completed within the past two years  * BLS certification (if not, training and certification will be provided upon hire and must be completed before providing direct patient care) * Bilingual Spanish/English * Strong phlebotomy skills  * Previous experience with Epic   Available Pay Differentials: $1.00 per hours worked for employees certified bilingual in Spanish, Chinese, or Tagalog.   Fair Chance Ordinance: Pursuant to the San Francisco Fair Chance Ordinance, we will consider for employment qualified applicants with arrest and conviction records.
