Employment Type: Part time Shift: Day Shift Description: Per Diem Social Worker - Hospice Albany The Community Hospice has a per diem opportunity for a social worker to join our hospice care team caring for patients in the Capital region as part of our Interdisciplinary Team in homes and Nursing homes. As a member of the hospice interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff. Position is Per diem Monday- Friday 8a-4:30p. Days are flexible. Responsibilities: Diagnose and treat psychological, behavioral and emotional disorders as they relate to end of life care. Provide specialized skills and mental health therapy for clients. Identify barriers which impact optimal patient functioning. Ensure patients receive appropriate resources in most appropriate setting. What you will need: NYS SW Licensure. Valid NYS Drivers license. Minimum 2-3 years experience ideally caring for seniors or those with advance illness. The Community Hospice provides a thorough orientation and a supportive work environment. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
