End Date: November 30, 2025
Schedule: Full-time, Mon–Fri, 8:00 AM – 4:30 PM (40 hrs/week)

JOB SUMMARY

The Day Center Personal Care Assistant (PCA) is responsible for delivering safe and compassionate personal care services to participants at the WelbeHealth Day Center. This role also supports Nutrition, Engagement (Activities), and Home Health departments as needed. The PCA works collaboratively with colleagues to promote WelbeHealth’s mission, values, and culture of care.

EDUCATION

  • High School Diploma or Equivalency (required, if not certified).

EXPERIENCE & SKILLS

  • Minimum 1 year of personal care experience with a frail or elderly population.
  • Experience using electronic medical records.
  • Strong organizational and communication skills.
  • Ability to follow instructions, work independently, and prioritize in a fast-paced setting.
  • Commitment to providing compassionate care and working with people from diverse backgrounds.
  • Reliable transportation (must be able to escort participants on WelbeHealth vehicles; participants will not ride in worker’s vehicle).

Responsibilities
  • Provide personal care and assistance with ADLs, including escorting, toileting, bathing, dressing, grooming, assisting with meals, and laundry.
  • Measure and document participants’ vital signs as instructed.
  • Assist participants with boarding/exiting transportation and act as a medical escort when needed.
  • Observe activity on the day center floor to ensure participant safety.
  • Assist with meal service and feeding support as required.
  • Support participant transfers in/out of homes and escort to appointments when assigned.
  • Maintain personal care supplies (incontinence, grooming items, etc.).
  • Assist Engagement team with activities and Home Care team as needed.
  • Clean participant areas between meals and activities.
  • Promptly report any changes in participant condition to the supervisor or IDT team.
  • Complete all required documentation accurately and on time.
  • Protect privacy and confidentiality of participants, families, and staff.
  • Participate in training, staff meetings, and quality improvement activities.
  • Communicate regularly with supervisor and team regarding workload and priorities.
