Description

The awarding winning proactive care team provides anticipatory care to adults with multiple long-term conditions in a defined area, through a combination of referral and case finding. The team seek to reduce inequalities in care and prevent unnecessary hospital admission and maximise patients’ potential through a multidisciplinary individualised approach.
The postholder will support the multidisciplinary team through social prescribing supporting patients within their own homes to build resilience through health and social support.

To work as part of a Proactive multi-disciplinary health team within a community setting to help people to identify issues that affect their health & wellbeing and co-produce a simple personalised care and support plan. Support people by connecting them to non-medical, community-based activities, groups and services that meet their practical, social and emotional needs, including specialist advice services and arts and culture, physical activity, and nature and green based activities
Use coaching and motivational interviewing techniques to support people to take control of their own health and wellbeing, identifying effective interventions, and liaison with relevant agencies. To develop excellent working relationships with other providers for this client group.
