REPORTS TO: MANAGER OF RESIDENCE

Since 1995, the compassionate caring staff and volunteers of Hospice Vaughan have supported individuals and their families, as they journey through a life-limiting illness. Serving with love and dignity, our commitment is to care for individuals to live fully and ensure a better ending.
Our new Hospice Palliative Care Hub includes a 12-bed hospice residence, providing a final peaceful homelike space for individuals and their families who require end of life support.
We are looking for a compassionate and dedicated Personal Support Worker (PSW) to join our team at the Hospice Residence, providing hands-on support and meaningful care to residents and their families.

POSITION REQUIREMENTS

· Successful completion of a recognized Personal Support Worker (PSW) program with valid certification.
· Completion of Fundamentals of Hospice Palliative Care or Advanced Palliative Practice Skills (APPS) considered an asset.
· Minimum of 2 years’ experience working as a PSW preferred.
· Recent experience in providing palliative/end-of-life care is an asset.
· Demonstrated client-centred approach to care, with sensitivity to the needs of clients and their families.
· Proven ability to work effectively within an interdisciplinary team.
· Self-motivated, compassionate, organized, and able to communicate effectively both verbally and in writing.
· Demonstrated ability to manage multiple priorities in a fast-paced environment while maintaining professionalism and empathy.
· Willingness to participate in mandatory in-service education/training sessions related to skill development, self-care, teamwork, and best practices.
· Proficient in basic computer use (e.g., electronic charting, email communication).
· Availability to work a variety of 8- and 10-hour shifts, including evenings, weekends, and holidays.
· Physically capable of performing the duties of the position, including standing, walking, climbing stairs, bending, reaching, and regularly lifting or moving up to 25 lbs.
All staff are required to receive the annual influenza (flu) vaccine as part of our commitment to protecting the health of residents, families, and the care team. Exceptions may be considered with appropriate medical documentation.
Police Check Requirements:

Responsibilities

JOB PURPOSE/SUMMARY:

The Personal Support Worker (PSW) will be responsible for providing assistance with daily living and end-of-life care to residents and their caregivers. The PSW will work as part of the interprofessional team to implement the plan of care, provide a home-like environment, and offer emotional, social, spiritual, and personal care that reflects the individual needs and preferences of each resident.
