Omnia Community Health was established through the merger in January 2026 of Sunbury and Cobaw Community Health and Nexus Primary Health. Bringing together the deep community connections, shared values and specialist expertise of both organisations, Omnia builds on more than 50 years of experience delivering health and wellbeing services within the not-for-profit sector.

We offer a broad range of services across the lifespan including general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing. These services are delivered from our sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach across surrounding communities.

About the Role

This role supports people to reduce social isolation, build meaningful connections, and participate actively in their community.

You will work alongside clients and team members in both centre-based and community-based group programs, as well as providing one on one support to individuals living in the community.

Our clients live locally and may be referred through My Aged Care, be participants of the National Disability Insurance Scheme (NDIS), or be funded through their Support at Home package.

To succeed in this role, you will need to build strong rapport with clients and carers, demonstrate excellent communication, planning, and problem solving skills, and be willing and able to drive a 12-seater bus.

Key Responsibilities

  • Sourcing, planning and delivering high quality, person-centred group activities in the centre and in the community.
  • Provide active supervision of clients during group activities.
  • Encourage client participation and inclusion, adapting activities to meet individual needs and abilities.
