Responsibilities

POSITION PURPOSE SUMMARY:

The Personal Support Worker (PSW) provides Resident Centered Care, based on the principles of treating residents with kindness, honesty, sensitivity, comfort, discretion, respect, and understanding. The PSW remains cognitive that all interactions follow the principles of preserving resident dignity, supporting independence, individualized care/preferences, privacy, and do no harm. The PSW works with the Registered Nurses’ direction and supervision, follows care plans, documents and reports to ensure continuity, and safe care.

Residents’ Nutritional Role: includes, but not limited to

  • Assists resident who require support and assistance eating
  • Setting up and cleaning up dining tables
  • Records food and fluid intake of residents
  • Distributes nourishment to resident

Residents’ Activation – PSW role includes, but not limited to

  • Range of motion exercises (as directed by Home Policy and Procedures where available)
  • Assisting with transfers, mobility, ambulation, walking
  • Use and care of adaptive aids/prosthesis
  • Transport/porter, accompany to activities, functions or internal appointments,
  • Maintenance & cleaning of assistive devices, transfer devices, and mobility aid

Communication & Documentation: The PSW’s role includes, but not limited to

  • Observes all aspects of the residents’ well-being (physical, emotional, spiritual), report to supervisor and update as needed, document appropriately, and monitor.
  • Implements directions of Registered Staff and Care Plan. Communicate changes in residents’ abilities and care
