SUMMARY The Pharmacy Technician supports the Care Navigation function by providing medication-related administrative and coordination support to the Care Team (RN, SW, Pharmacist) in service of DSNP members. Working directly with members, members’ families and/or advocates, providers, pharmacies, and community partners, the Pharmacy Technician helps ensure members receive timely, safe, and affordable medications and pharmacy services that promote medication adherence, quality of life, and achievement of member-identified goals. The Pharmacy Technician will empower members to understand and effectively use their prescription benefits, identify and address medication access barriers, and advocate on behalf of members when appropriate. This role assists the care team with medication reconciliation, medication adherence outreach, formulary navigation, prior authorization coordination, and documentation related to medication management. The Pharmacy Technician communicates internally and externally to coordinate medication-related tasks, track outcomes, and support quality and compliance initiatives tied to DSNP and CMS requirements. ESSENTIAL JOB DUTIES AND RESPONSIBILITIES Orient new members to pharmacy-related benefits within the Belong Health DSNP program, including formularies, preferred pharmacies, mail order, and cost-sharing. Conduct medication outreach activities to support adherence, refill coordination, medication synchronization, and resolution of access issues. Assist with medication reconciliation by collecting and documenting current medication lists and identifying discrepancies for review by the RN or Pharmacist. Provide input to the interdisciplinary care team regarding medication-related needs, barriers, and opportunities to support the member’s individualized care plan (ICP). Coordinate with pharmacies, prescribers, and internal teams to support prior authorizations, formulary exceptions, refills, and transitions of care. Identify and resolve non-clinical medication-related issues (e.g., pharmacy access, coverage questions, refill delays) and escalate clinical concerns to licensed staff. Support members in accessing medications, supplies, and pharmacy services, including coordination of delivery, durable medical equipment related to medications, and over-the-counter benefits when applicable. Assist the care team with documentation related to medication management, care coordination activities, and interdisciplinary care team meetings. Educate members, families, and caregivers on medication benefits, safe medication practices, and available pharmacy support resources within scope of practice. Ensure accurate data entry, tracking, and follow-up of medication-related tasks and member outreach activities. Support compliance with CMS, Medicare, and Medicaid quality measures related to medication adherence and appropriate medication use. Promote a culture of accountability and performance to meet or exceed service expectations and ensure timely resolution of complex, non-routine medication access issues. SUPERVISORY RESPONSIBILITIES This role does not have supervisory responsibilities. REQUIRED PROFESSIONAL EXPERIENCE AND QUALIFICATIONS 2+ years of experience as a Pharmacy Technician or in a medication-related role within a healthcare, managed care, pharmacy, or clinical support environment. Current Pharmacy Technician certification and/or state registration as required. Experience working with Medicare Part D and/or Medicaid pharmacy benefits strongly preferred. Understanding of the needs of elderly, disabled, and underserved populations, including individuals with complex medication regimens. Strong problem-solving and organizational skills with the ability to manage multiple medication-related tasks simultaneously. Commitment to high ethical standards; protects the privacy of member and Belong data and exercises discretion in handling confidential information. Excellent oral and written communication skills with members, providers, and pharmacies. Demonstrated PC skills across multiple platforms and applications. Strong attention to detail and follow-through skills. Ability to work independently and collaboratively within an interdisciplinary care team. Familiarity with healthcare quality and performance measures, including HEDIS, NCQA, CMS Medicare Star Ratings, and pharmacy-driven quality and adherence programs. PREFERRED EDUCATION AND TRAINING Completion of an accredited Pharmacy Technician training program. Familiarity with coordinated care models and value-based care environments, including ACOs, DSNPs, and contractual arrangements with third-party payors. WORK ENVIRONMENT Remote, telehealth-based work environment with standard office responsibilities, including prolonged periods of sitting, computer use, and frequent electronic documentation. Requires a secure, private workspace that supports confidentiality and compliance with HIPAA and organizational data security standards. Technologically proficient and self-sufficient in a remote setting, with the ability to independently set up, maintain, and troubleshoot required hardware, software, and secure systems. Ability to work independently while adhering to established clinical, operational, and regulatory workflows in a fast-paced, regulated healthcare environment. Reliable, high-speed internet is required to support secure communications, telehealth operations, and system access.
