Description

Family Health Teams (FHTs) use a patient-centered approach to primary health care that brings together doctors, nurses, nurse practitioners, registered dietitians, pharmacists, mental health clinicians and other health care professionals and support staff to co-ordinate the highest possible quality of care for patients – when they need it, as close to home as possible. FHTs are a key component of Ontario’s healthcare plan to reduce wait times, improve access to primary care, and keep Ontarians healthy.

PATIENT AND CAREGIVER EDUCATION

  • Provide education on safe and effective medication use, potential side effects, and adherence strategies.
  • Support patients and caregivers with self-management strategies, including lifestyle modifications (e.g., diet, physical activity, smoking cessation).
  • Deliver group and individual health education sessions to enhance chronic disease self-management.

QUALIFICATIONS

  • Bachelor of Science in Pharmacy from a recognized university
  • Registration with the Ontario College of Pharmacists (Part A)
  • Two years’ experience preferred, or community/hospital residency and/or Pharm D an asset: involvement in Primary Care preferred
  • Additional training or credentialing in pharmacotherapy (eg. Certified Diabetes Educator, PHARM-MOAT – Management of Oral Anticoagulant Therapy Course from U of Waterloo, CAMH TEACH certification) will be necessary depending on the specific pharmacist’s role.
  • Additional training or credentialing in pharmacotherapy (eg. Certified Geriatric Pharmacist, Certified Asthma Educator, Hypertension etc) considered an asset.
  • Ability to work both independently and effectively in an interdisciplinary team environment
  • Experience in providing patient-centred care and evidence-based unbiased drug information
  • Proof of professional liability insurance
