Description PURPOSE STATEMENT Pharmacists assess, monitor, initiate and modify medication use according to patient condition and age specific criteria. Pharmacists ensure prompt and accurate medication preparation and dispensing. Pharmacists supervise and direct supportive pharmacy personnel. ESSENTIAL FUNCTIONS Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems. Recommend formulary alternatives when appropriate. Compound and dispense medications for accurate delivery to patients. Provide drug information and education to medical staff and patients. Communicate with physicians and other clinicians as necessary to facilitate care. Oversee technical personnel who assist in the preparation, maintenance and dispensing of medications and supplies. Provide patient counseling efficiently and effectively when required. Adhere to policies, procedures and regulations to ensure compliance and patient safety. Maintain competence required for position. Perform other related duties as assigned. Requirements EDUCATION/EXPERIENCE/SKILL REQUIREMENTS High school diploma or equivalent. Graduate from an accredited School of Pharmacy with a Bachelor of Science degree, or Doctorate of Pharmacy degree. Missouri Board of Pharmacy pharmacist licensure with Medication Therapy Services designation in good standing. Must have attention to detail, excellent customer service skills, excellent verbal and written communication skills. Must have the ability to work independently as well as part of a team, ability to prioritize and problem solve, ability to mathematically compute and convert drug concentrations and doses. Previous hospital experience preferred. Completion of an ASHP-Accredited PGY1 Residency preferred. PHYSICAL/MENTAL REQUIREMENTS Must be able to sit and stand, intermittent 8 to 10 hours a day. Must be able to use standard office equipment, including the telephone and computer keyboard. Continuously works under pressure of near 100% accuracy while meeting inflexible deadlines. Continuously utilizes manual/bi-manual dexterity, near vision, speech, and hearing. Frequently stands, walks, sits and utilizes eye/hand coordination and color definition. Occasionally reaches above shoulder, regularly required to life and/or carry up to 40 lbs. Occasionally walks on uneven surfaces.
