POSITION SUMMARY:

As a key member of Gateway Community Health Centre’s interprofessional team, the Pharmacist plays a vital role in supporting client-centered care through medication management, chronic disease support, transitions in care, and staff education. The Pharmacist will contribute to improved health outcomes by ensuring safe, effective, and appropriate use of medications, and providing timely support across primary care services.

EDUCATION AND PROFESSIONAL SUPPORT

• Deliver staff education on new medications, vaccine updates, or changing clinical guidelines.
• Lead or contribute to in-service training for clinical staff.

QUALIFICATIONS:

• Licensed Pharmacist in good standing with the Ontario College of Pharmacists (OCP).
• Minimum of 2 years’ experience in primary care, hospital, or community pharmacy settings.
• Experience with chronic disease management and medication reviews.
• Familiarity with Telus EMR or similar electronic medical record systems.
• Strong communication and collaboration skills within a team-based care model.
• Comfortable with client-centered care, home visits, and outreach when needed.
