Description
The Pharmacist is a member of the multidisciplinary healthcare team that provides pharmaceutical care to patients at Hamilton Health Sciences. The Pharmacist reviews and assesses medication therapies and works with the patient and the healthcare team directly to optimize medication use and minimize medication-related problems to achieve desired patient outcomes. This work may be performed on individual clinical units, or may be across multiple units to support the medication system needs of the site. 1. Licensed or eligible for license by the Ontario College of Pharmacists Part A of the Registry 2. Baccalaureate of Science in Pharmacy or Doctor of Pharmacy Degree 3. Demonstrated computer skills including pharmacy systems, word processing, spreadsheet, and e-mail systems 4. Demonstrated ability to function as an independent practitioner 5. Demonstrated innovation and project leadership skills 6. Demonstrated commitment to continuous learning 7. Demonstrated excellence in independent practice, teamwork, change management, teaching, problem-solving and communication skills 8. Hospital Pharmacy Residency preferred
