Responsibilities

You’ll deliver clinical pharmacy services, support patient care, and collaborate closely with an experienced multidisciplinary team. This supportive environment will allow you to sharpen your clinical skills and grow in confidence as a healthcare professional.

Responsibilities

  • Provide efficient clinical pharmacy services to inpatients and/or outpatients.
  • Conduct pharmaceutical admission and discharge reconciliations, monitor medication outcomes such, as therapeutic drug monitoring, to ensure quality use of medicine is upheld.
  • Work cooperatively and effectively within a multidisciplinary team and provide advice regarding patient care.

Shift Information

  • Tuesday to Friday, 3:30pm – 8pm.
