The pharmacist on duty provides many services, with the assistance of technical and clerical support. This is includes preparation and distribution of medications, providing drug information, education, inventory management, order verification, patient profile review and monitoring, discharge instructions, various medication dosing services, antimicrobial stewardship, interdisciplinary discharge planning, and committee representation. Responsibilities · Ongoing commitment to reducing costs and improving productivity in the pharmacy department. · Dispenses medications accurately and efficiently for inpatients and outpatients. · Maintains positive interpersonal skills with patients, family, physicians, managers, staff, community and all constituencies. · Maintains inventory of supplies and reorders supplies accordingly · Performs quality control activities on schedule · Assesses medication use for all ages and health conditions. · Monitors drug therapy and provides drug information per Scope of Service. · Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. · Participates in all aspects of TRMC collaborative practice initiatives, including but not limited to formulary substitutions, kinetic dosing and drug dosing per medical staff orders. · Assists Pharmacy Manager with training of technicians-in-training and pharmacist interns · Plays an active role in antimicrobial stewardship activities, including regular outreach calls with Infectious Disease staff. · Performs appropriate documentation of interventions, adverse drug reactions, medication errors, and other documentation as required by the Pharmacy Manager and regulation Qualifications Education: B.S. Degree in Pharmacy, Doctor of Pharmacy preferred Experience: Experience with progressive responsibilities and demonstrated leadership abilities in hospital Pharmacy preferred. License(s)/Certification(s): Current license to practice Pharmacy in the State of Iowa Valid driver’s license when driving any vehicle for work-related reasons.
