ABOUT THE ROLE

An exciting opportunity has arisen for a dynamic and motivated Band 7 Pharmacist to join our dedicated pharmacy team. This role offers the chance to play a crucial part in ensuring the safe and effective use of medicines at both ward and dispensary levels, in adherence to Trust policies, pharmacy procedures, local and national guidelines, and legal requirements.

KEY RESPONSIBILITIES

As a Band 7 Pharmacist, you will be an integral part of the multidisciplinary team (MDT), providing expert clinical pharmacy input to ensure optimal patient care. Your responsibilities will include:

  • Offering specialist advice to clinicians regarding medicines, including unlicensed medicines and areas with limited evidence bases, liaising with Medicines Information Pharmacists when necessary.
  • Delivering a high-quality clinical pharmacy service to patient groups, ensuring effective treatment outcomes with medicines.
  • Monitoring patients for potential and actual adverse effects of medicines, intervening as required.
  • Ensuring the accurate, timely, and efficient supply of medicines for inpatient use and appropriately labelled medication for discharge.
  • Supporting discharge planning, including liaising with community team pharmacists to facilitate follow-up care where appropriate.
  • Providing expert assessments of medicine regimens during enquiries and routine ward visits, offering specialist advice on complex medicine-related issues such as drug interactions, high-dose antipsychotic prescribing, and unlicensed medicine use.
  • Prescribing medicines within your scope of practice, in accordance with Trust policy, if qualified and registered as an Independent Pharmacist Prescriber with the General Pharmaceutical Council (GPhC).
  • Supporting the implementation of Trust Medicines Management Policies within your designated clinical area, ensuring medicines are handled safely and securely.
  • Engaging in ongoing professional development to maintain and enhance your clinical knowledge and competencies.
