Conduct medication therapy management services to enhance patient safety and adherence. Doctor of Pharmacy (Pharm.D.) degree from an accredited institution. Active pharmacist license in the state of practice. Previous experience as a pharmacist is preferred. Strong knowledge of pharmacology, medication management, and drug interactions. Excellent communication and counseling skills to educate patients on medication usage. Ability to work collaboratively in a multidisciplinary healthcare team.
