Primary Responsibilities Dispense prescription medications accurately and provide medication counseling to patients. Review medication orders and patient profiles to identify potential drug interactions or adverse effects. Collaborate with healthcare professionals to develop individualized medication treatment plans. Monitor patient responses to drug therapy and adjust medications as needed for optimal outcomes. Maintain medication inventory and ensure compliance with regulatory standards. Stay updated on pharmaceutical knowledge and industry trends to provide the highest quality of care. Conduct medication therapy management services to enhance patient safety and adherence. Doctor of Pharmacy (Pharm.D.) degree from an accredited institution. Active pharmacist license in the state of practice. Previous experience as a pharmacist is preferred. Strong knowledge of pharmacology, medication management, and drug interactions. Excellent communication and counseling skills to educate patients on medication usage. Ability to work collaboratively in a multidisciplinary healthcare team. Detail-oriented with good organizational and time-management skills. Commitment to ongoing professional development and continuous learning in the field of pharmacy.
