QUALIFICATIONS:

Required:

  • Completion of course at an accredited School of Pharmacy, with a degree of Bachelor of Science in Pharmacy or PharmD

Preferred:

  • Hospital pharmacy experience

SCHEDULING REQUIREMENTS:

  • Shift working, rotating schedule of day shift and evening shift with occasional overnight shifts as needed for coverage
  • Weekend and Holiday rotation
  • Per Diem

Essential Functions:

  • Filling and verifying Pyxis refills
  • Dispensing floor stock items according to approved lists and par level quantities
  • Verifying and preparing aseptic IV admixtures, TPN, and compounded products following good manufacturing pharmacy practice
  • Evaluating medication therapy for appropriateness and taking corrective action when needed
  • Serving as a drug information resource for healthcare professionals
  • Reviewing medication orders for proper dosing and identifying interactions or allergies
  • Promptly clarifying medication orders with physicians or nurses as needed
  • Documenting interventions for therapy changes and ensuring pharmacy personnel are informed
  • Promoting rational medication therapy based on current literature, hospital guidelines, and clinical observation
