EMPLOYMENT REQUIREMENTS:

Education/Experience:
* Doctor of Pharmacy or
* Bachelor of Science in Pharmacy with Accredited Canadian Pharmacy Residency (ACPR) or
* Bachelor of Science in Pharmacy with 2 years of relevant acute care hospital pharmacist experience
* 2 years minimum Experience in Oncology setting preferred
* Must be Registered Pharmacist Part A with Ontario College of Pharmacists
* Experience with Pharmaceutical care, Medication Reconciliation and Hazardous Sterile Compounding preferred
* Clinical Pharmacist Specialist preferred however if there are no qualified candidates, Clinical Pharmacist with an
Honours Bachelor of Science in Pharmacy will be considered

Salary Range: $65.300 to 69.150

Looking for exceptional clinically practicing pharmacists to join our Inpatient Pharmacy Team!
Thunder Bay Regional Health Sciences Centre (TBRHSC) is a 423 bed acute care teaching hospital. TBRHSC
provides care to patients from across Northwestern Ontario. The Clinical Pharmacist Specialist (CPS) will be
responsible for clinical pharmacy key performance indicators including but not limited to medication reconciliation,
developing and initiating pharmaceutical care plans, resolving drug therapy problems, participation in
interprofessional patient care rounds and collaboration, patient education, and dispensary related work. CPS will be
committed to quality improvement processes, mentorship, education and perform critical decision making regarding
medication management. The CPS will be under the general supervision of the Pharmacy Manager.
Under the general supervision of Pharmacy Manager, Clinical Pharmacist Specialist (CPS) will be responsible for
assisting in the therapeutic review of chemotherapy drug orders and ensure they are evidence based, assistance in
building of care plans in Mosaic, oral chemotherapy teaching and other assigned duties. In addition, the Pharmacist
is actively involved on interprofessional teams to provide exceptional care to patients.
Preference will be given to Clinical Pharmacist Specialists, however if there are no qualified applicants, Clinical
Pharmacists with a Bachelor of Science in Pharmacy will be considered.
