Job Summary:
We are seeking a dedicated Pharmacist to join our team. The ideal candidate will provide exceptional pharmaceutical care to patients, ensuring safe and effective medication use.
Responsibilities:
- Conduct patient assessments and provide medication therapy management
- Collaborate with healthcare providers on patient care plans
- Perform utilization reviews to optimize medication regimens
- Utilize medical management principles to enhance patient outcomes
- Demonstrate proficiency in aseptic technique for sterile compounding
- Provide pharmaceutical care in critical care, pediatrics, and acute pain management settings
- Offer patient-centered services focusing on individual needs
Experience:
The successful candidate should possess the following skills and qualifications:
- Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program
- Active pharmacist license in good standing
- Completion of a pharmacy residency program preferred
- Experience in critical care, patient assessment, and utilization review
- Proficiency in aseptic technique and sterile compounding
- Strong background in pediatrics and acute pain management
- Excellent patient service skills with a focus on patient care
