EXPERIENCE YOU WILL NEED: PHARMACY CLINICAL SPECIALIST - HEMATOLOGY/ONCOLOGY

  • Graduate of an education program approved by the credentialing body for the required credentials.
  • Successful completion of an ASHP-accredited post-graduate year one (PGY1) residency training program or equivalent experience, such as three years of pharmacy experience per residency year.
  • ASHP-accredited post-graduate year two (PGY2) or fellowship preferred.
  • Three years of clinical practice experience.
  • Availability Monday through Friday from 8:00 AM to 4:30 PM, with rotating on-call responsibility.PHRM - Pharmacist - State Licensure

    • Texas Pharmacist License or eligible for Texas Pharmacist License within 90 days AND

    • PPC- Pharmacist Preceptor Cert - State Licensure within 90 days

    Responsibilities
    • Perform advanced clinical pharmacy duties to enable optimal pharmacotherapy delivery to patients.
    • Facilitate safe, efficacious, and pharmacoeconomically rational use of medications in the population(s) served.
    • Conduct daily reactive and targeted proactive medication regimen reviews for patients.
    • Support the delivery of pharmacy managed protocols, formulary processes, and medication reconciliation activities.
    • Provide education to fellow pharmacists, nurses, and medical staff.
    • Precept pharmacy students and pharmacy residents on clinical patient care rotations.
    • Contribute to fulfilling the organization’s mission of providing innovative, personalized, and cost-effective pharmaceutical care.
