Location: Sentara Obici Hospital, Suffolk VA
Status: Full Time (40 hrs/wk)

EDUCATION

  • Bachelors Level Degree in Pharmacy (Required)
  • Doctorate Level Degree in Pharmacy (Preferred)

EXPERIENCE

  • Minimum 3 years of Hospital Pharmacy Experience (Required)
  • Previous Leadership Experience (Preferred)
Responsibilities
  • Manages patient drug therapy in accordance with applicable clinical practice standards
  • Assures the appropriateness of all provider medication orders in maintaining a high level of patient care and safety
  • Maintains advanced knowledge of medication use, disease states, and regulations applicable to their area of practice
  • Oversees the training of new employees and the creation and maintenance of training materials
  • Assists with the development of policies and procedures
  • Demonstrates superior customer service, effective communication, decision-making, and organization
  • Triages and escalates any questions or issues outside the scope of practice as needed
  • Uses appropriate systems for processing of patient information
  • Ensures quality assurance is maintained according to departmental and legal requirements
  • Precepts pharmacy students and/or pharmacy residents as assigned
  • Provides leader and peer coaching and constructive feedback to colleagues in a professional and productive manner
  • Successfully leads local and system implementation of various system initiatives
  • Develops, implements, and evaluates pharmacy services, protocols, policies & procedures consistent with established standards
  • Participates in local and/or system committees related to pharmacy services as necessary and assigned
  • Provides operational support and staffing based on the needs of the team
  • Serves as a liaison for provider and community-relations activities
  • In addition to the above job responsibilities, other duties may be assigned
  • Valid pharmacist license required in the state of practice
