Summary The Pharmacy Technician supports pharmacists, patients, and providers by ensuring accurate medication reconciliation for individuals within the Curana Health census. This reconciliation process is a key component of delivering personalized care to our Chronic Care Management residents and patients. Through this work, we verify the accuracy of each patient’s documented medication regimen in the EMR while also identifying potential drug interactions, dosage issues, and opportunities for de-prescribing. Essential Duties & Responsibilities Complete the Pharmacist CCM Medication Reconciliation Template in the EMR, including: Comprehensive medication reconciliation and documentation of changes since the last review Medication adherence assessments, including identifying barriers and outlining plans to address concerns Drug–drug interaction reviews, including identifying medications that may require changes or monitoring and documenting supporting rationale Identification of potentially inappropriate medications (PIMs) and fall-risk medications, with recommendations for change or monitoring Identification of deprescribing opportunities, including recommended approach and timeline Collaborate closely with pharmacists and the CCM care team to support patient safety and quality of care Receive, verify, and enter prescription orders into the EMR/pharmacy system Provide phone-based support to patients, including answering questions and sharing medication-related information under pharmacist supervision Support insurance-related processes, including updating pharmacy information, assisting with claims, and supporting prior authorizations Maintain accurate and timely documentation of patient profiles, prescriptions, and controlled substances Ensure compliance with all federal and state regulations, including HIPAA and controlled substance requirements Perform other duties as assigned to support program operations Qualifications Strong attention to detail and commitment to accuracy Clear, professional communication skills and ability to work effectively with patients, pharmacists, CCM team members, and providers Comfort using pharmacy systems, EMRs, and medical terminology, with the ability to quickly learn new tools (e.g., ThoroughCare, GehriMed) Strong organizational and time-management skills in a fast-paced, high-volume environment Pharmacy Technician certification preferred Education & Experience High school diploma or equivalent required Associate’s degree preferred 2+ years of experience as a pharmacy technician PTCB national certification preferred Work Environment Hybrid (office and remote) Occasional travel to select assisted living or long-term care facilities Pay Range USD $20.00/Yr. - USD $23.00/Yr.
