Overview Promotes optimal patient outcomes by assisting with medication reconciliation. Interviews patients and reviews medication-related documentation for the purpose of compiling a medication history for patients being admitted to the hospital. Compiled history information is entered into the patient’s electronic health record for the purpose of facilitating medication reconciliation by the admitting provider. May perform a variety of functions which impact the medication reconciliation process and patient care. Seeks assistance/clarification from a pharmacist with any questions or issues that may arise. Responsibilities 1. Interviews patients or caregivers to collect information regarding all current home medications;identifies when a discrepancy related to inappropriate dosing, frequency, or schedule for a homemedication exists, and uses this knowledge to obtain clarification to ensure accuracy of the homemedication list and to facilitate the timely/accurate collection of data.2. Utilizes appropriate resources to collect information from various sources (patient interviews,physician offices, retail pharmacies, family members, historical encounters, extended carefacilities, examination of medicine bottles, etc.) regarding home medications to ensure a completeand accurate home medication list.3. Utilizes designated computer applications to accurately enter information collected from interviewactivities for the generation of admission medication reconciliation documentation; this informationincludes, but is not limited to, correct medication name, medication formulation, dose, route,directions, schedule, and date/time of last dose taken. Produces documents for subsequentphysician review.4. Records the patient’s preferred outpatient pharmacy in the electronic medical record.5. Contacts and communicates with other care providers (pharmacists, nurses, physicians, etc.) asneeded to facilitate the processing of home medication information in an efficient manner.6. Follows up on outstanding issues from the previous shift requiring further phone calls orpatient/family interview and communicates any delays/difficulties in the reconciliation process toappropriate disciplines.7. Consults with the pharmacist in all cases of unresolved discrepancies related to a patient’smedication history.8. Willingly helps teach new personnel, residents, and students.9. Recognizes when to consult with the pharmacist for doubts or questions that require clinicaljudgment.10. Ability to set priorities in correct order in relation to the needs and circumstances at the time withreference to the patients, co-workers, physicians, and the nursing unit. Qualifications License, Certification and/or Registration: • Must be registered with the Alabama State Board of Pharmacy as a Pharmacy Technician within 30 days of hire date, and registration must be in good standing “active”. • National certification with PTCB (Pharmacy Technician Certification Board) or ExCPT (Exam for the Certification of Pharmacy Technicians) required. • In the case of PTCB or ExCPT renewal, “active” status must be attained within 30 days of the expiration of the current certification (this provides a 30-day grace period for the employee). Education Required: High school diploma or GED required. Experience: Experience applying customer service behaviors and communication skills required. Combination of both inpatient and outpatient experience preferred, particularly outpatient/retail experience. About Us DeKalb Regional Medical Center celebrated its 35th anniversary in October 2021.
