To undertake patient facing and patient support roles to ensure effective medicine use Carry out medicines optimisation tasks in accordance with PCN and practice protocols to update patients medical records / prescribing accordingly To support the achievement of the practices prescribing Quality and Outcomes Framework (QoF) targets Supporting the delivery of specifications that relate directly to the Investment & Impact Funding system Working with the Clinical Pharmacists and Prescription Clerks to contribute to the review and development of repeat prescribing protocols to improve the efficiency and effectiveness by championing electronic prescriptions To support the Clinical Pharmacist to provide medication reviews, patient information leaflets, medicine awareness; and to undertake community visits as appropriate.
