Responsibilities

PURPOSE OF JOB:

To provide clients with a daily/twice daily telephone call to support them to maintain independence in their own home and offer a reassuring call. This role will be carried out from our offices.

MAIN RESPONSIBILITIES:

  • Contribute to the day-to-day operation of the service as a member of the Contact Team.
  • To make telephone contact with the client group daily/twice daily.
  • Monitor and record the outcome of each telephone call, update systems of any relevant observations and note and report to the manager all matters relevant to the client’s health and wellbeing.
  • Ensure client records are maintained to service standards.
  • Develop positive relationships with clients and their families/carers.
  • Assist clients in signposting information about links to services in their local communities.
  • To liaise with service staff if client requires further assistance.
  • To follow the Organisation’s policies and procedures.
  • Maintain and update client database and records ensuring all records and information are up to date.
  • To participate with other team members in team meetings to review and improve the service.
  • Ensure equal opportunities are always implemented.
  • Participate in all relevant training as deemed necessary.
  • Any other reasonable duties that the Manager or CiC Board may determine in support for the service.
