Responsibilities:

  • Average of 8 patients per day; no nights or weekends
  • Ability to work as a team member to improve the health of the people and communities we serve
  • Provider will comanage patients with Physician, assist with Inpatient Acute Rehab Unit follow-up appointments, as well as patients with a range of areas including musculoskeletal pain, deficits from neurologic or orthopedic conditions, amputee care, deconditioning, and other similar conditions.
  • Improve quality of life by optimizing performance and functionality
  • Evaluates, interprets and coordinates information to make decisions and recommendations regarding patient care
  • Patient-focused, friendly and willing to collaborate in a team environment
  • Excellent communication (written and verbal), analytical and problem-solving skills

Qualifications:

  • Nurse Practitioner or Physician Assistant
  • Board Certified
  • Hold an unrestricted medical license in the state of Iowa
  • Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
  • Eligible to work in the United States
  • An interest in overall wellness, weight loss, nutrition, and exercise is preferred
