How you’ll make an impact in this role Evaluate patients with physical, emotional, cognitive, and/or psychosocial deficits. Design and implement patient-centered, effective treatment plans using appropriate modalities. Assess the patient's ability to safely perform independent living skills. Document patient progress, monitor, and modify the plan of care as necessary. Plan and assist with discharge, including education and recommendations for adaptive equipment. What minimum requirements you’ll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required. A valid license pending letter will be accepted until license is issued. Education: Bachelor's degree required. Master's degree preferred. What additional preferences we're seeking 1-year acute care experience.
