Summary This position evaluates and treats patients with neurological, musculoskeletal and/or integumentary pathologies and/or other acute or chronic physical ailments through the use of skilled physical therapy techniques used to relieve pain, restore physical functioning and promote independence. The position also frequently involves patient and family education, and advocacy for adaptive equipment when needed, to support the prevention of disabilities and/or reoccurrence of injury. Essential Duties and Responsibilities * The following information is intended to be representative of the work to be performed by incumbents in this position and is not all-inclusive. Provide therapeutic treatments and evaluation of equipment needs as applicable. Develop and revise the plan of care in consultation with the physician and other care-team members. Evaluate the home environment and make recommendations for safety or other adaptations. Conduct initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time points. Establish plan of care and goals based on the referral and evaluation of functional skills; joint mobility; gait; developmental reflexes; muscle strength, tone, and coordination; sensation tests; and other measures to help ensure rapid patient recovery. Implement therapy according to the established plan, utilizing appropriate procedure, equipment and techniques within the scope of physical therapy. Observe and report findings about the patient’s condition to the attending physician and other agency staff, and document information in the patient’s record Potential participation in wound management through wound debridement, dressing changes, exercise, stretching and splinting. Recommend adaptive equipment, complete the equipment justification forms and arrange for the delivery of equipment. Evaluate outcomes of care and assess patient progress and make recommendations regarding discharge. Consult and co-treat with other patient care team members to discuss patient progress, set objectives for patient treatment and integrate physical treatment with other activities. Professional and appropriate communication with other disciplines providing care. Establish and maintain effective communication and a professional relationship with client, families, and coworkers. Educate the patient and/or the caregiver about the findings of the evaluation, established goals of the physical therapy program, plan of care, home programs, injury prevention and other related information. Prepare clinical and progress notes in alignment with agency policies. Evaluate home and/or work environments and makes physical modifications or ergonomic recommendations. Provide written instructions for the home health aide to promote re-enforcement of therapy services Perform supervisory visits with or without physical therapist assistant or home health aide’s presence at least every 10th visit if there is a designated therapy skill being performed in the home. Understand all treatment and services being supervised. Understand responsibilities associated with each level of care provided by staff under Therapist supervision Potential assistance with departmental program development and mentoring physical therapy students. Participate in and provide in-services for staff, other departments and the community. Provide documentation of patient care according to established standard criteria. Participate in continuous quality improvement. Supervise the patient care activities of physical therapy assistants and rehabilitation aides. Participate in medical conferences, continuing education courses and other related activities. Attend case conference and staff meetings. Take part in other duties and meetings that may be assigned by the Vice President of Home Care or Home Care Clinical Administrator. Wear assigned identification while providing patient care. Practice in a manner consistent with the professional and Harmony code of ethics. Comply with all organizational policies regarding ethical business practices Follow Harmony’s adverse event/incident reporting procedures and Emergency Preparedness Plan. Assure continued competency by maintaining and updating professional performance, knowledge and skills. Communicate and emulate the mission, ethics and goals of the Harmony Home Health & Hospice. Submission of clinical documentation, visit notes and daily time summaries/time sheets is required for satisfactory completion of an assignment as defined by Company policy. Maintain confidentiality in relation to all patient/clients, healthcare staff and documentation. Seeks direction from the VP of Home Care/designee when/if asked to share any patient/client information with outside parties. Utilize universal precautions and follow OSHA guidelines at all times. Assignments are fulfilled with reliability and punctuality. Maintain a cooperative manner towards client/family and all members of the healthcare team. Qualifications Qualifications and Experience Bachelor’s Degree or higher from an accredited school of Physical Therapy. Licensed as a Physical Therapist in the practicing state. Prior experience in Home Health preferred, but not required. Effective written and oral communication skills and good interpersonal skills. Current driver’s license, auto insurance, safe driving record and reliable transportation. Current CPR certification, TB test, and other compliance as required by the agency. Adherence to ethical guidelines as outlined by the American Physical Therapy Association and Harmony. Criminal background check before hire. Satisfactory passing of a skills evaluation test Meet continuing education requirements of the practicing state Maintain patient confidentiality at all times. Demonstrate effective time management and organizational skills; provide patients visits as scheduled; complete patient care within the designated time. Physical demands: Physical Therapists under the employment of Harmony Home Health Care are required to perform job related duties which may include: lifting, standing, bending, transferring, stooping, stretching, walking, pushing, pulling, talking, hearing, and the ability to provide partial or complete assistance with activities of daily living without assistance from another healthcare worker or caregiver. The employee must occasionally lift and/or move up to 60 pounds without assistance, and help to support and/or transfer patients who potentially weigh much more than 60 pounds, with physical assistance or the aid of assistive devices. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Work environment: While performing the duties of this job, the employee is exposed to the weather conditions which are prevalent at the time. The noise level in the work environment is usually minimal.
