All Care and Comcare Home Health is currently seeking a qualified Physical Therapist to join our dedicated healthcare team. This position offers a unique blend of administrative oversight and hands-on patient care, presenting an excellent opportunity for a PT looking to expand their professional scope. Position: Field Physical Therapist Key Responsibilities: * Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders. * Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC). * Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals. * Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals. * Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens. Qualifications: * Active license/registration with the Physical Therapy Board of California. * Graduate of an accredited Physical Therapy program. * Strong documentation and organizational skills. * Excellent communication and interpersonal abilities. * Willingness to travel to patient homes for evaluations and treatment. * Flexibility regarding work locations throughout our service area.
