JOB DESCRIPTION

We are actively interviewing Physical Therapist Assistants to work Full-time in our skilled nursing community to implement individualized therapy programs designed to help restore and enhance our patient’s ability and provide information and instruction to patients and caregivers on necessary skills for maintaining independence.

REQUIREMENTS

• Graduate of accredited Program
• Current state licensure

• Implementing PT treatment for patients with various conditions.
• Monitor patient progress and document changes in their health status.
• Provide support in skilled nursing facilities and home environments.
• Educate patients and families on therapeutic exercises and activities to decrease caregiver dependence and increase safety.
• Collaborate with healthcare teams to develop and adjust care plans as necessary.
• Maintain a safe and clean environment for patient care.
