Description POSITION OVERVIEW Improving the quality of life through prescribed exercise, hands-on care, and patient education, by examining each person and then developing a treatment plan to improve their ability to move, reduce or manage pain, restore function, and prevent disability. ESSENTIAL FUNCTION Performs and documents daily assessments of patient’s condition Follows a plan of care which is appropriate to problems in identified and involves the patient/family according to their capabilities and desires Educates the patient and family/caregiver about patient deficits. Plans and/or assists with patient discharge from therapy services and treatment settings. Provides information regarding appropriate selection/use of adaptive equipment and support programs. Within scope of job, requires critical thinking skills, decisive judgement, and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.
