We're looking for a dedicated Physical Therapist Assistant (PTA) to join our dynamic care team. In this role, you'll work under the guidance of a licensed Physical Therapist, delivering personalized treatment plans that help patients recover, maintain, and improve their physical function.   Key Responsibilities: * Implement individualized treatment plans using a variety of therapy techniques and modalities * Monitor patient progress and communicate updates or adjustments to the supervising therapist * Document care services and patient outcomes with accuracy and clarity * Educate patients and their caregivers about physical challenges and therapeutic goals * Support discharge planning, including equipment recommendations and community resource referrals * Collaborate with the healthcare team to ensure the highest quality of patient care * We're looking for someone with strong communication skills, clinical insight, and a heart for patient-centered care.
