AvaMed Inc. is a leading Healthcare Staffing Agency specializing in placing qualified providers throughout California State Correctional Facilities. We are currently seeking a Physical Therapist for a full-time assignment at the correctional facility in Stockton, CA. Job Details: Physical Therapist Full-Time Stockton, CA Monday - Friday; 7:30am - 4:00pm Weekly Pay $90/hr as a 1099/ Independent Contractor Job Duties/Requirements: Perform patient care within the scope of licensure, following Institution/Facility policies, procedures, and evidence-based Physical Therapy practices. Provide Physical Therapy treatments and evaluations on an as needed basis as scheduled by the Institution/Facility CEO, CME, or designee. Deliver Physical Therapy services to patient/youth populations based on: Physician referral and order, Direct access requests for an existing and documented diagnosis. Conduct initial assessments/evaluations for each referral and develop a treatment plan aligned with the findings. Communicate treatment plans clearly to the patient/youth and relevant medical staff. Complete all reports, assessments/evaluations, and progress notes using approved documentation forms within the patient chart or outpatient health record. Remain available for telephone consultations while on duty at the Institution/Facility. Maintain daily charting; no chart may remain delinquent for more than 10 days. Responsibilities
