AvaMed Inc. is a leading Healthcare Staffing Agency specializing in placing qualified providers throughout the California State Correctional Facilities. We are currently seeking a Physical Therapist for an assignment at the correctional facility in Stockton CA. Must have a minimum of 1 year experience. Job Details: Physical Therapist Stockton, CA Monday - Friday 8:00pm - 4:30pm (4/10's schedule available) Orientation is 1 week Monday - Friday 7:00am - 3:30pm Weekly Pay $85/hr 1099 Rate (W2 and Travel rate available) Job Duties/Requirements: Perform care consistent with the scope of licensure and in accordance with the Institution/Facilities policies and procedures and consistent with generally accepted evidence based Physical Therapy practices. Care provided must be sufficiently complex and consistent with the unique skills of a physical therapist. Provide Physical Therapy and evaluations on an "as needed" basis on patient/youth as pre-scheduled by designated Institution/Facility CEO/CME or designee. Provide Physical Therapy services to patient/youth upon physician referral and order, or upon direct access to the Physical Therapy services in cases where the patient/youth is seeking Physical Therapy for an existing and documented diagnosis. Provide an initial assessment/evaluation on each referral and develop a treatment plan consistent with those findings and be responsible for communicating the treatment plan to the patient/youth and any relevant medical personnel. Reports or assessments/evaluations and progress notes shall be completed utilizing approved documentation forms in the patient's chart and/or outpatient health record. Vendor Worker shall be available for telephone consultation while on duty at the Institution/Facility. Maintain all charting on a daily basis. Any chart remaining delinquent for more than ten days. Responsibilities
The Physical Therapist will provide care consistent with their licensure and the facility's policies, performing evaluations and developing treatment plans for patients. They will also maintain documentation and be available for telephone consultations while on duty.
