About the Role: As a Physical Therapist in the Health Care Services industry, you will play a critical role in improving the quality of life for patients through personalized rehabilitation programs. You will assess patients' physical conditions, develop tailored treatment plans, and implement therapeutic interventions to restore mobility, reduce pain, and prevent disability. This role requires working closely with patients in various settings, including home health environments, to ensure continuity of care and optimal recovery outcomes. You will collaborate with multidisciplinary teams to monitor progress and adjust treatments as necessary, ensuring evidence-based practices are followed. Ultimately, your expertise will empower patients to regain independence and enhance their overall physical function. As a Physical Therapist, you will: * Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention. * Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients. * Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.  * Applies concepts of infection control/universal precautions in performing patient care * Assure continuity of quality patient care delivered with appropriate documentation * Provide effective communication to patient/family, team members and other health care professionals * Promote agency philosophy and administrative policies * Maintain and complete all documentation within company stated guidelines.  Requirements * Successful completion of accredited physical therapy program and state licensure exam. * Valid Physical Therapy license * 1 year of home health experience preferred * CPR certification * Valid Driver’s license * Valid Auto Insurance * HomeCare HomeBase experience preferred Physical Demands * Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods. * Must be able to lift, carry, pull, or push up to 100 lbs..
