Your Role as a Physical Therapist As a Physical Therapist with Concierge Home Care, you will deliver personalized, patient-centered care while leveraging advanced technology and AI-supported tools designed to streamline documentation and improve efficiency. Key Responsibilities: * Travel to patients’ homes to provide individualized physical therapy services that promote recovery, mobility, and independence * Conduct comprehensive evaluations to assess patient conditions and rehabilitation needs * Develop and implement goal-oriented treatment plans in collaboration with patients, caregivers, and the interdisciplinary care team * Utilize AI-assisted documentation and charting tools to support timely, accurate note completion and faster turnaround times * Guide and supervise Physical Therapy Assistants to ensure high-quality, compliant care delivery Our technology-forward approach allows you to spend more quality time with patients and less time charting, supporting better work-life balance.
