Responsible for a clinical caseload and provides direct care to clients. Reports directly to Manager. Plans and delivers community therapy services to individuals, families and groups. Completes a needs analysis for individuals and groups within designated areas of the SHA. Based on this needs assessment, plans, develops and delivers treatment programs. This may include providing direct therapy to clients but often involves training and consulting to other caregivers who provide direct patient care. Organizes community therapy services in a designated area in accordance within the philosophy, objectives and policies of the community therapy program and the SHA. Human Resources Exemption: No Experience * 2 years of relevant experience Education * Bachelor of Science in Physical Therapy Knowledge and Abilities * Strong initiative and independent judgement to solve complex problems * Initiative & judgment to plan implement/modify treatment programs for clients * Ability to plan, organize/coordinate efforts with other team members * Demonstrates responsible care and use of equipment and supplies * Ability to identify equipment repair needs and monitor supplies for ordering * Ability to promote a collaborative and team approach * Develops and implements a life-long post-graduate education plan * Ability to work independently and in collaboration with others as required * Promote professional development of staff and engender job satisfaction * Ability to be proactive and creative in decision making * Ability to integrate ideas into a team treatment approach * Ability to adapt to changing conditions * Ability to assist clients dealing with stress, role loss, and other psych issues * Responsible for accurate observation/documentation of patient related info.
