Deliver evidence-based therapy services that promote resident mobility, function, independence, and quality of life. Maintain timely and accurate documentation to meet reimbursement and regulatory requirements. Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate care. Educate residents, families, and team members regarding treatment plans and wellness strategies. Support residents in achieving their personal goals while honoring their preferences and routines. Responsibilities

Evaluate and implement individualized physical therapy treatment plans to promote resident mobility and independence. Collaborate with an interdisciplinary healthcare team to coordinate care and educate residents and their families.
