JOB OVERVIEW

We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. This role involves working with clients in home health environment. The Physical Therapist will be responsible for developing individualized care plans and implementing therapeutic interventions tailored to each client’s needs.

SKILLS

  • Strong knowledge of anatomy and physiology relevant to physical therapy practices.
  • Proficient in medical terminology related to patient assessments and treatments.
  • Experience in pediatrics, geriatrics, orthopedics, acute care, and outpatient settings is highly desirable.
  • Familiarity with home & community care practices is an asset.
  • Ability to work effectively with diverse patient populations and adapt treatment approaches accordingly.
  • Excellent communication skills for educating clients and collaborating with healthcare professionals.

QUALIFICATIONS:

  • Bachelor’s or Master’s degree in Physical Therapy from an accredited program
  • Valid state licensure as a Physical Therapist
  • Anatomy knowledge and understanding of human physiology
  • Familiarity with medical terminology and documentation practices
  • Experience working in a hospital or acute care setting is preferred
  • Experience with pediatric patients is a plus
  • Strong interpersonal and communication skills
  • Ability to work effectively as part of a multidisciplinary team

    Pay: $120.00 - $145.00 per hour

Responsibilities

DUTIES

  • Conduct comprehensive evaluations to assess clients’ physical abilities and limitations.
  • Develop and implement personalized treatment plans based on assessment findings.
  • Provide hands-on physiotherapy treatments, including orthopedic rehabilitation.
  • Collaborate with interdisciplinary teams to ensure coordinated patient care.
  • Educate clients and their families on treatment plans, exercises, and self-management strategies.
  • Monitor client progress and adjust care plans as necessary to achieve optimal outcomes.
  • Document all client interactions and treatment sessions accurately in EMR system.
  • Participate in discharge planning to ensure smooth transition from home setting to outpatient therapy.

PRINCIPAL RESPONSIBILITIES:

  1. Provide Initial and ongoing assessment to determine level of functioning.
  2. Gather background data, reviews referrals, selects and modifies evaluation tools, and evaluates clients determining the level of client’s physical and psychosocial functioning.
  3. Develop and revise the plan of care in consultation with the physician and other care-team members.
  4. Be responsible for planning, evaluation, and treatment implementation of assigned caseload.
  5. Prepare clinical progress notes.
  6. Supervise therapy assistants and home health aides as appropriate.
  7. Evaluate outcomes.
  8. Educate clients and/or families about proper and safe use and recommend methods for installation of adaptive equipment.
  9. Assist client in accomplishing treatment plan and accept therapeutic devices by administering manual exercises, instructing, encouraging, and assisting clients in performing physical activities, such as nonmanual exercises, ambulatory functional activities, and daily living activities and in using assistive and supportive devices such as crutches, canes and prostheses.
  10. Evaluate the effects of physical therapy treatments and fit of devices, recommend adjustments and modifications.
  11. Assure continuation of therapeutic plan following discharge by designing home exercise program.
  12. Maintain client confidence and adhere to HIPAA requirements.
  13. Protect clients and employees by adhering to infection-control policies and protocols.
  14. Maintain safe and clean environment by complying with procedures and regulations.
