Rivers Health is seeking a full-time Physical Therapist to perform the duties below:       JOB SUMMARY:     Entails initial evaluation and re-assessment of patients referred to the department, planning and administrating treatments and documentation of patient’s status and progress.   JOB RELATIONSHIPS: Responsible to:  Director of Rehabilitation Nature of supervision received:  Indirect Positions directly supervised:  Physical Therapy Assistants, Rehab Techs, Students, and Volunteers.   ESSENTIAL JOB FUNCTIONS: The following is a summary of the major job functions of this job.  The employee may perform other duties, both major and minor, which are not mentioned below; and specific functions may change from time to time. 1. Maintain and upgrade evaluation and treatment skills. 2. Evaluate patient's problems subjectively and objectively and document data via SOAP format, set goals, plans, and implement treatment program. 3. Assess patient progress and alter treatment plan as needed. 4. Provide accurate and professional documentation as required by departmental policy. Documents patient's status and progress in accordance with departmental policy. 5. Initiate physical therapy services upon physician referral and guidance of qualified physician. 6. Must submit progress/discharge reports to physicians and other referral sources on a regular basis. 7. Provide patient care utilizing physical agents and therapeutic exercise upon referral and guidance of qualified physician. 8. Provides supervision for P.T.A.'s, rehab techs, students, and volunteers. 9. Assist with orientation and training of new personnel. Interact with other team members to provide total care of patients. 10. Participates in staff meetings and in-services. Keeps up to date in the profession through literature, workshops, seminars, etc. Responsible for professional growth. 11. May function as clinical instructor for physical therapy students. 12. Demonstrates excellence in customer service skills. 13. Foster an atmosphere of teamwork and collaboration. 14. Participates in quality improvement activities including serving on quality improvement teams. 15. Maintain cardiopulmonary resuscitation certification. 16. Performs other related or similar duties as may be periodically assigned. 17. Aware of and adhere to departmental policies and procedures, as well as hospital policies and procedures. 18. Responsible for identifying and addressing the needs of patients of all ages including pediatrics, adolescents, and geriatrics.   Qualifications   QUALIFICATIONS: The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this job.   EDUCATIONAL AND FORMAL TRAINING:   Four or five years of formal education and Bachelor/Master of Science degree or equivalent in Physical Therapy from an accredited institution. Current West Virginia Physical Therapy License.
Responsibilities
