Identify barriers to functional independence and design individualized physical therapy programs. Address challenges related to mobility, posture, gait, and injury or illness recovery. Provide interventions to help residents regain strength, prevent falls, and maintain independence. Support residents in daily functional tasks, recommending adaptive equipment as needed. Conduct exercises and mobility programs to enhance coordination, flexibility, and overall physical function. Assess living spaces to recommend modifications for safety and accessibility. Educate residents, families, and staff on therapy goals, safe mobility, and fall prevention. Collaborate with OTs, SLPs, nursing, and interdisciplinary teams to ensure holistic care. Maintain accurate records of assessments, treatments, and progress, ensuring compliance with healthcare regulations and community policies.
