QUALIFICATIONS

• Graduate of an accredited Physical Therapy program with current licensure in California.
• Minimum of one year of clinical physical therapy experience; home health experience preferred.
• Strong clinical assessment and observational skills.
• Excellent communication and patient education abilities.
• Valid California driver’s license and an insured vehicle in good working order.

As a Physical Therapist (PT) with Guardian Angel in California, you’ll play an essential role in providing high-quality, personalized rehabilitative care to patients in their homes. Responsibilities include:

• Conducting initial patient evaluations and creating tailored treatment plans.
• Delivering physical therapy interventions to restore functional abilities.
• Educating patients and caregivers on therapeutic exercises and safety.
• Coordinating with the care team and overseeing PTAs as needed.
• Documenting progress and ensuring compliance with CMS, CHAP, and California state standards.
• Participating in an on-call rotation when required.
• Supporting continuous quality improvement through chart audits and outcome assessments.
