Key Responsibilities: • Provide effective patient care using approved therapy techniques and procedures • Evaluate patient condition and establish measurable treatment goals • Develop and implement individualized treatment plans • Supervise and coordinate care with PTAs and HHAs • Document patient progress, updates, and treatment notes accurately and on time • Educate patients and families on exercises, mobility, and use of adaptive equipment • Evaluate home environments and recommend modifications to improve safety and independence • Ensure all services meet HIPAA standards for confidentiality • Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care Qualifications Qualifications: • Active Physical Therapist (PT) license in Arizona • Minimum 1 year of PT experience preferred • Physical exam within the last 6 months • Current TB test (within 12 months) and CPR certification • Valid driver’s license and reliable transportation Available Opportunities: We offer full-time, part-time, and per diem roles in AZ, CA, CT, FL, IL, IN, MA, MN, NV, and OH.
