Job Summary We are seeking a caring and motivated Physical Therapist (PT) to join our Home Health team in Northern Oakland County. This role is ideal for PTs who enjoy flexibility, independence, and one-on-one patient care. The PT will perform evaluations, develop treatment plans, and help patients improve mobility, strength, and overall function within the comfort of their homes. Why Join Safe Hands? Competitive per-visit rates: $80 per Evaluation and $65 per Revisit Flexible scheduling within your preferred coverage areas Supportive clinical and administrative teams Clear communication, reliable caseload support, and organized processes Work with an experienced and respected home health provider Responsibilities Perform physical therapy evaluations and reassessments in the patient’s home Develop and update individualized treatment plans in collaboration with physicians Provide therapeutic exercises, mobility training, and functional rehabilitation Educate patients and caregivers on safety, mobility, and home exercise programs Document all visits and progress within the agency’s EMR system Coordinate with nurses, aides, and other interdisciplinary team members to ensure continuity of care Maintain compliance with agency, state, and federal guidelines Qualifications Qualifications Active and unrestricted Physical Therapist (PT) license in Michigan Minimum 1 year of recent PT experience (Home Health experience preferred) Strong clinical judgment and communication skills Reliable transportation and a valid driver’s license Compassionate and patient-focused approach to care EMR experience preferred Background Screening Safe Hands conducts background screenings on all candidates through the Michigan LARA system once a contingent offer is made.
