Our San Ramon-based team is growing, and we’re seeking a dedicated Physical Therapist (PT) to help clients restore mobility, improve strength, and enhance their overall quality of life. In this role, you’ll develop and implement personalized therapy plans, collaborate closely with caregivers and clinical staff, and ensure every client receives seamless, individualized care that supports their recovery and independence. Position Summary We are seeking an experienced, motivated Physical Therapist (PT) to join our home health team. You’ll provide skilled therapy services to clients recovering from surgery, illness, or injury—all from the comfort of their homes. Your assessments and individualized treatment plans will help patients restore mobility, independence, and quality of life. Key Responsibilities Perform initial evaluations and re-assessments to determine mobility, balance, and functional needs Develop and implement customized treatment plans aligned with patient goals Deliver gait training, range-of-motion exercises, and fall prevention strategies Educate patients and caregivers on home safety, adaptive techniques, and equipment use Collaborate with PTAs, RNs, OTs, and interdisciplinary team members during care coordination Accurately document services in the EMR (WellSky) per Medicare guidelines Monitor patient progress and communicate changes in condition to the care team Supervise PTAs and aides as needed Qualifications Active, unrestricted California Physical Therapist license Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited program Successful completion of NPTE and California Law Exam Minimum 1 year of clinical experience (home health or outpatient preferred) Current CPR certification Proficient in electronic documentation (WellSky, Kinnser, or similar) Reliable transportation and valid California driver’s license Excellent interpersonal and clinical reasoning skills Benefits Flexible scheduling for PTs seeking work-life balance Access to clinical and administrative support from Case Managers and office staff Participation in recognition programs (e.g., Clinician of the Week and Clinician of the Month) Referral bonuses for bringing in other licensed clinicians Paid Time Off (PTO) available to Per Diem staff after completing 720 hours of service Why Join Qualicare?
